Referrals to MYINDSET 

Thank you for trusting MYINDSET with your patient’s behavioral health care. We work closely with primary care physicians, OB/GYNs, pediatricians, therapists, and other healthcare providers throughout central Indiana—including Hendricks County, the greater Indianapolis area, Delphi, and the greater Lafayette region—to ensure coordinated, comprehensive treatment. With three office locations and telehealth available statewide, we can accommodate referrals from across Indiana.

For Healthcare Providers

If you are a provider looking to refer a patient to MYINDSET for psychiatric evaluation, medication management, perinatal mental health services, therapy, please submit referrals by fax at (855) 869-2201 or by secure email at referral@myindset.com

Please include the patient’s name and date of birth, contact information, insurance information, reason for referral and relevant clinical history, and any current medications.

Our team will follow up with the patient to schedule their initial appointment. Typical wait times and availability may vary depending on the service requested.

If you need a referral as a current patient, please contact our office at 317-207-0273 and we’ll be happy to assist you.

If you are a new patient and your provider has recommended you contact MYINDSET, you can get started right away by clicking the Establish Care button below to complete your prospective patient intake form. You do not need a formal referral from your doctor to schedule an appointment with us.

Services Available for Referral

Psychiatric evaluations and medication management (depression, anxiety, ADHD, PTSD, bipolar, OCD, substance use), Spravato (esketamine) treatment for treatment-resistant depression, perinatal and postpartum mental health with integrated lactation support, individual therapy and counseling, and peer recovery coaching for mental health and substance use.
